Please read below for important guidelines about making appointments
We have been given interim guidelines from the CMTBC, RMTBC and WCB on how to reopen in a safe manner.
If you have had any air travel, or been to another province we ask that you wait 14 days before coming in for an appointment.
Please do not arrive prior to 5 minutes before your appointment start time.
The waiting room will have a sign posted on the limit of how many people can be waiting at one time, please observe and follow what is requested. If the door is locked, your therapist will let you in when they are ready to.
You will be required to immediately go wash your hands for a minimum of 20 seconds upon your arrival.
We have strict cleaning, disinfecting, and sanitizing protocols in place and have increased the time between appointments to ensure adequate time to do so.
We request that you wear a mask or covering at all times while in the clinic.
It is mandatory to wear a mask or covering while at the front desk and face up during your treatment.
If you do not have a mask or covering, one will be provided to you for the cost of $1.
Refusal to wear a mask or covering in the mandatory areas will result in asking you to leave the premises.
Existing and new clients will have to fill out a new informed consent form, this must be filled out prior to starting your appointment. This will be emailed to you.
New Patients please fill out an online Medical History Form.
An additional email will be sent to you once you book your appointment with a link to fill out the intake form. If you do not see it in your inbox, it could be in your spam folder. Please note that if you have been to the southwest Main and 16th clinic, you will need to fill out another form. The records for both clinics will be separate. If you are booking a Beauty Room appointment, the intake form does not need to be filled.
Please note that while the booking system does confirm and remind you of your appointment, it is the ultimate responsibility of the client to keep track of their appointment. Not receiving a reminder email is not a valid reason for missing an appointment.
Please be advised that a minimum notice of one (1) full day is required for any cancellation. In the event that you must reschedule or cancel with notice less than 24 hours, you will be responsible for the full cost of that time slot.
